Flu Vaccinations for Non-Employed MLH Providers. Important Information to Access the ReadySet Platform.
Providers,
It’s that time of year again, Influenza Vaccination, to be sure that you can receive your vaccine from MLH, you will need to be sure that you have created your username and password in ReadySet. You should have received an email to the email address you provided Workday from our ReadySet platform requesting you to create an account.
To create your account from that email (if you have not already done so or do not have single sign on capability) you will need the following information:
The email address to which this email is addressed
Org code: 3773
DOB: 01/01/1900 (use this as your DOB)
Social Security number: 999-99-9999 (use this as your SSN)
This will allow you to set up a username and password for your ReadySet account, please be sure to keep up with that information for future log in. This will be necessary to receive the flu vaccine and document any fit testing that you have completed.
All Associates and Providers must receive the flu vaccine by Oct. 31, 2026, or have an approved religious or medical exemption on file with Associate Health. All new Exemption request are due by 4 p.m., Oct. 15, 2026. Please allow 5-7 business days for review of documentation by an Associate Health team member and approval decision, checking back to ensure whether your exemption request has been accepted or denied after this review period is encouraged to ensure approval, denied request will be expected to receive the vaccine by Oct. 31, 2026, or further documentation may be required.
Got your flu shot outside of MLH? Proof of vaccines received outside of MLH MUST be uploaded with your health survey (questionnaire) in the ReadySet system by Oct. 31. Emailed documentation will no longer be accepted. Once you have submitted outside flu vaccine documentation in ReadySet, please check back within 5-7 business days to ensure that your documentation has been accepted by a member of the Associate Health team. Documentation must contain the following information for it to be accepted: Provider name, date of birth, type of vaccine administered, date of administration, lot number of vaccine, expiration date of vaccine, and name of doctor’s office, pharmacy, or other location where the vaccine was administered to you.
