Reappointment Information
Reappointment is required bi-annually for Medical Staff and Allied Health Professionals.
Prior to expiration, you will receive a notice indicating that re-application is required. Complete the application and request for clinical privileges and return within 30 days to ensure that privileges will not expire. Failure to reapply in a timely manner will be an automatic voluntary resignation from the medical staff. Resignation requires an initial application to regain membership and/or clinical privileges.
